To maintain your professional liability insurance coverage, please complete your Renewal Application annually.
Only include addenda, questionnaires, or additional forms if they were required for your renewal last year, or if an Underwriter has asked you to provide them. You’ll receive a renewal reminder 60 to 70 days before your policy expires.

First-Time Application for Professional Liability Insurance
Complete this form (and the CASL form) to purchase professional liability insurance for the first time as a new Certificate of Practice holder resulting from a merger, acquisition, or new license from the OAA.
